Looking back, I can see that February is always a time of ?home.? ?I seem to be planning for family visits, organizing or just playing ?Susie Homemaker.? Here?s my page from three years ago.

Winter X Games snowmobiler dies after Colo. crash

(AP) ? Caleb Moore was a Texas kid drawn to the snow, rehearsing complicated tricks on a snowmobile into a foam pit back home until they became second nature and ready for the mountains.

With his younger brother following along and constantly pushing him, Moore became a rising talent in action sports.

The innovative freestyle snowmobile rider, who was hurt in a crash at the Winter X Games in Colorado, died Thursday morning. He was 25.

Moore had been undergoing care at a hospital in Grand Junction since the Jan. 24 crash. Family spokeswoman Chelsea Lawson confirmed his death, the first in the 18-year history of the X Games.

"He lived his life to the fullest. He was an inspiration," Lawson said.

A former all-terrain vehicle racer, Moore switched over to snowmobiles as a teenager and quickly rose to the top of the sport. He won four Winter X Games medals, including a bronze last season when his younger brother, Colten, captured gold.

Caleb Moore was attempting a backflip in the freestyle event in Aspen last week when the skis on his 450-pound snowmobile caught the lip of the landing area, sending him flying over the handlebars. Moore landed face first into the snow with his snowmobile rolling over him.

Moore stayed down for quite some time, before walking off with help and going to a hospital to be treated for a concussion. Moore developed bleeding around his heart and was flown to a hospital in Grand Junction for surgery. The family later said that Moore, of Krum, Texas, also had a complication involving his brain.

Colten Moore was injured in a separate crash that same night. He suffered a separated pelvis in the spill.

The family said in a statement they were grateful for all the prayers and support they have received from people around the world.

X Games officials expressed their condolences and said Moore, a four-time X Games medalist, would be remembered "for his natural passion for life and his deep love for his family and friends."

The Twitter accounts of many athletes from a wide range of sports expressed their sympathy Thursday:

B.C. Vaught, Caleb Moore's agent for almost a decade, said he first saw Moore when he was racing an ATV in Minnesota and signed him up to star in some action sports movies.

Later, Moore wanted to make the switch from ATVs to snowmobiles and Vaught helped him. A natural talent, it only took Moore two weeks to master a difficult backflip.

Moore honed his skills in Krum, a town about 5,000 people 50 miles northwest of Dallas that rarely sees snow. Instead, he worked on tricks by launching his sled into a foam pit. After a brief training run on snow ramps in Michigan, he was ready for his sport's biggest stage ? the 2010 Winter X Games.

In that contest, Moore captured a bronze in freestyle and finished sixth in best trick. Two years later, his biography on ESPN said, "Caleb Moore has gone from 'beginner's luck' to 'serious threat.'"

That was hardly a surprise to Vaught, who said, "Whatever he wanted to do, he did it."

Vaught said Moore didn't believe his sport was too extreme, but rather "it was a lifestyle." He was good at it ? along with ATV racing ? as he accumulated a garage full of trophies.

Still, Moore's death is sure to ignite the debate over safety of the discipline. Whether action sports are too dangerous is an issue that's been raised before.

When freestyle skier Sarah Burke died in a training accident a little more than a year ago in Park City, Utah, there were questions about the halfpipe. Before that, the sport was examined when snowboarder Kevin Pearce suffered a severe brain injury in a fall in the same pipe as Burke two years earlier. Pearce has recovered and served as an analyst at Winter X.

But in general, the athletes accept the risks and defend their disciplines.

"I just look at it like this: Yes, we're in a dangerous sport," fellow snowmobile rider Levi LaVallee said. "Anytime you're doing a backflip on anything, it's dangerous. But we're training to do this. This is what we practice, what we do day in and day out. We're comfortable with doing this stuff."

LaVallee recently described Moore as a "fierce competitor."

"A very creative mind," LaVallee said. "I've watched him try some crazy, crazy tricks and some of them were successful, some of them not so much. But he was first guy to get back on a sled and go try it again. It shows a lot of heart."

X Games officials said in a statement that they would conduct a thorough review of freestyle snowmobiling events and adopt any appropriate changes.

"For 18 years, we have worked closely on safety issues with athletes, course designers and other experts. Still, when the world's best compete at the highest level in any sport, risks remain," they said, noting that Moore was hurt performing a move he had done several times before.

Several governors tackle early childhood education, but not Gov ...

Unlike Gov. Jerry Brown, several governors have started the year highlighting early childhood education programs, according to a story in Education Week.

Govs. Deval Patrick, D-Mass., Rick Snyder, R-Mich., and Mike Pence, R-Ind., all used January speeches to propose expansions of early childhood education programs in their states from state preschool to full-day kindergarten. Govs. Peter Shumlin, D-Vt., and Neil Abercrombie, D-Hawaii, also talked about the importance of investing in early childhood education in January speeches, according to the Education Week story.

Govs. John Hickenlooper, D-Colo., and Dannel Malloy, D-Conn., also have proposed expanding preschool programs.

In contrast, Brown didn?t mention early childhood education at all in his budget address or in his State of the State address. He hasn?t proposed cuts to early education and child care spending, nor has he proposed expansions. As EdSource reported earlier this month, funding for early childhood programs would stay flat in the governor?s proposed budget.

However, early childhood programs in California have been cut by about $1 billion since the 2008-09 fiscal year and now serve 110,000 fewer children than they did before the recession, according to the Legislative Analyst?s Office. ?Preschool advocates wish the issue would move higher up on the governor?s agenda.

Scott Moore, policy adviser for Preschool California, an advocacy organization that pushes for expanded early learning options in the state, said he applauds the governor?s focus on K-12 education but would like ?to see a broadening of that focus to include preschool. State Superintendent of Public Instruction Tom Torlakson has also highlighted early childhood education as an area that deserves more funding in his responses to the governor?s January speeches.

Gene finding may lead to treatments effective against all MRSA strains

Jan. 31, 2013 ? In the last decade, a new strain of MRSA has emerged that can spread beyond hospital walls, putting everyone at risk of contracting the dangerous bacterial infection. This particular strain of MRSA -- known as USA300 -- contains a chunk of genes not shared by any other strains, though it is unclear how this unique genetic material enables the bacteria to survive and persist in the community.

Now, research from the University of North Carolina School of Medicine has pinpointed a gene that causes the infection to linger on the skin longer than other strains, allowing it to be passed more readily from one person to the next.

The gene makes this strain of MRSA resistant to specific compounds on the skin called polyamines that are toxic to other forms of the bacteria. In uncovering this property, researchers have identified a novel target for developing new treatments against methicillin-resistant Staphylococcus aureus, particularly the USA300 strain that accounts for the vast majority of MRSA skin and soft tissue infections seen in emergency rooms.

"The problem is by the time you figure out how one strain comes into dominance, it often fades away and a new strain comes in. But because these compounds occur naturally and are so toxic, we still think they can lead to treatments that are effective against all MRSA. We will just have to put in a little extra work to block the gene and make this particular strain of MRSA susceptible to polyamines," said senior study author Anthony Richardson, PhD, assistant professor of microbiology and immunology at the UNC School of Medicine.

The UNC study, published Jan. 16, 2013, in the journal Cell Host & Microbe, follows an attribute of MRSA previously unexplored by other scientists -- its sensitivity to the naturally occurring compounds called polyamines.

Polyamines are critical to wound repair because they are anti-inflammatory and promote tissue regeneration. Scientists first observed that MRSA infections were killed by polyamines in the 1950s, but no one followed up until recently, when Richardson decided to twist this scientific observation into a treatment option.

He and his colleagues tested hundreds of MRSA strains and found that all of them except one -- USA300 -- were sensitive to polyamines. When they looked to see what was different about this particular strain, they found that it contained a chunk of 34 genes, called the arginine catabolic mobile element (ACME), that none of the other strains possessed.

Then the researchers mutated each of these genes, one by one, until they created a strain that could be killed off by the polyamines. To confirm that they had the right gene, the researchers added a normal, non-mutated version of the gene -- named SpeG -- to other strains of MRSA and showed that it could make them resistant to these compounds.

Finally, Richardson wanted to know if the gene exerts the same effects in the context of a real infection. Using mouse models of MRSA infection, he and his colleagues showed that the presence of the SpeG gene helped the potent USA300 strain to stay on the skin for anywhere from a day to a week, giving the infection time to spread to the next host.

"Previously, the field tried to understand MRSA by focusing on attributes that we already knew were important, such as the amount of toxins or virulence factors a given strain makes. Those elements may explain why the disease is so bad when you get it, but they don't explain how a particular strain takes over. Our work uncovers the molecular explanation for one strain's rapid and efficient spread to people outside of a crowded hospital setting," said Richardson.

Prostate cancer study tracks long-term urinary, sexual and bowel function side effects

Jan. 30, 2013 ? A new study comparing outcomes among prostate cancer patients treated with surgery versus radiotherapy found differences in urinary, bowel and sexual function after short-term follow-up, but those differences were no longer significant 15 years after initial treatment.

The study, led by first author Matthew Resnick, M.D., instructor in Urologic Surgery, Vanderbilt University Medical Center, was published in the Jan. 31 issue of the New England Journal of Medicine.

From Oct. 1, 1994, through Oct. 31, 1995, investigators enrolled men who had been diagnosed with localized prostate cancer in the Prostate Cancer Outcomes Study (PCOS).

For the current study, investigators followed 1,655 men between the ages of 55 and 74 from the PCOS group, of whom 1,164 (70.3 percent) had undergone prostatectomy, while 491 (29.7 percent) had undergone radiotherapy. At the time of enrollment, the patients were asked to complete a survey about clinical and demographic issues and health-related quality of life. The men were contacted again at set times following treatment and were asked about clinical outcomes and disease-specific quality of life issues.

Men whose prostates had been surgically removed were significantly more likely than those who received radiation therapy to report urinary leakage at two years and five years. However, at 15 years, the investigators found no significant difference in the adjusted odds of urinary incontinence. Nonetheless, patients in the surgery group were more likely to wear incontinence pads throughout the 15-year follow-up period.

Men in the prostatectomy group were also significantly more likely than those in the radiotherapy group to report having problems with erectile dysfunction two years and five years after surgery.

"At the two- and five-year time points, men who underwent prostatectomy were more likely to suffer from urinary incontinence and erectile dysfunction than men who received radiation therapy," said Resnick. "While treatment-related differences were significant in the early years following treatment, those differences became far less pronounced over time."

Despite early and intermediate-term data revealing treatment-dependent differences in patterns of sexual dysfunction, after five years both groups had a gradual decline in sexual function. At 15 years, erectile dysfunction was nearly universal with 87 percent in the prostatectomy group and 93.9 percent in the radiotherapy group reporting sexual difficulties.

The authors noted that age may have played a role in the patients' waning sexual function, as shown in unrelated studies.

Some patients also experienced problems with bowel function in the years following treatment. Those who were treated with radiotherapy had more problems in the short term. Men in the radiotherapy group reported significantly higher rates of bowel urgency than those in the prostatectomy group at two years and five years.

However, at 15 years, despite absolute differences in the prevalence of bowel urgency between the two groups, the researchers found no significant difference in the odds of bowel urgency. Men who had been treated with radiotherapy were significantly more likely to report being bothered by bowel symptoms at both the two-year and 15-year points.

"This study of 15-year outcomes represents a mature portrait of quality of life issues following prostate cancer treatment," said David Penson, M.D., MPH, Ingram Professor of Cancer Research, professor of Urologic Surgery and Medicine, and director of the Vanderbilt Center for Surgical Quality and Outcomes Research, the senior study author.

"Regardless of the form of initial treatment, patients in this study had significant declines in sexual and urinary function over the duration of the study. The causes of these declines probably include advancing age and additional cancer therapies, in addition to the original therapy," Penson said.

"Patients need to be aware that all aggressive therapies for prostate cancer have significant side effects and perhaps these data make an argument for active surveillance (avoiding aggressive treatment and closely following the cancer) in certain cases."

Since the median life expectancy after treatment for prostate cancer is 13.8 years, the authors suggested that these data may be used by physicians to counsel men who are considering treatment for localized disease.

New study sheds light on link between dairy intake and bone health

Feb. 1, 2013 ? A study by researchers at the Institute for Aging Research (IFAR) at Hebrew SeniorLife, an affiliate of Harvard Medical School (HMS), has found that dairy intake -- specifically milk and yogurt -- is associated with higher bone mineral density (BMD) in the hip, but not the spine. Cream, on the other hand, may be associated with lower BMD overall. Published February 1 in the journal Archives of Osteoporosis, these findings suggest that not all dairy products are equally beneficial in promoting bone strength.

"Dairy foods provide several important nutrients that are beneficial for bone health," says lead author Shivani Sahni, Ph.D., Musculoskeletal Research Team, IFAR. "However, cream and its products such as ice cream have lower levels of these nutrients and have higher levels of fat and sugar. In this study, 2.5 -- 3 servings of milk and yogurt intake per day were associated with better bone density. More research is needed to examine the role of cheese intake (some of which can be high in fat and sodium), and whether individual dairy foods have a significant impact in reducing fractures."

IFAR researchers based their findings on data collected from a food frequency questionnaire completed by 3,212 participants from the Framingham Offspring study. They then compared participants' dairy intake with BMD measurement, which revealed the benefits of milk and yogurt versus cream in largely middle-aged men and women. According to the study, nutrient composition varies among dairy foods. Choosing low-fat milk or yogurt over cream can increase intake of protein, calcium and vitamin D while limiting intake of saturated fats.

This study is an example of a growing area of research focused on the relationship between nutrition and bone health. Past studies suggest that dairy products contain more than one beneficial nutrient, and for this reason certain dairy products may contribute towards maintaining healthier bones.

Research like this supports the idea that proper nutrition can help combat osteoporosis and fractures. Osteoporosis is considered a major public health threat for an estimated 44 million Americans, or half of those aged 50 and older.

? An estimated 10 million in the U.S. already have the disease. Women are at higher risk than men.

? Another 34 million Americans have low bone density, putting them at increased risk for osteoporosis and fractures, especially of the hip, spine and wrist. About one-quarter of those who suffer a hip fracture die within a year of the injury.

? Osteoporosis-related fractures were responsible for an estimated $19 billion in health care costs in 2005, with that figure expected to increase to $25 billion by 2025.
